Community Supports Aide job description

The Youth Peer Specialist (YPSS) will act as a support and resource for youth with serious emotional disturbances (SED) who are receiving services in the behavioral health system. The YPSS act as a key member of the treatment team by working with youth one on one or in a group, supporting them in achieving goals through shared activities and interventions. YPSSs will support youth empowerment by assisting youth in developing skills to improve their overall functioning and quality of life and working collaboratively with others involved in delivering the youth’s care.

High school diploma or GED; Young adult, ages 18 through 26. Experience receiving services as a youth in complex, child serving systems (behavioral health, child welfare, juvenile justice, special education, etc.) preferred. Willing and able to self-identify as a person who has or is receiving mental health ervices as a youth and has lived experience and is prepared to use that experience in helping others. Valid driver’s license.  Knowledge of resources in child serving systems and how to access useful resources in the community.

Provides assessment, diagnosis, treatment, and care management services to individuals and groups relating to mental illness and or emotional problems in a community based treatment setting. JOB DUTIES: The major function of this position is to coordinate the activities and services provided by the ACT IDDT Team, and to provide assessment, linking, monitoring, and treatment planning services to ACT IDDT individuals. The Team Leader’s goal is to see that each individual’s individualized needs are met in the least restrictive manner; also provides group, individual or family therapy with ACT IDDT individuals on an as-needed basis. QUALIFICATIONS: Registered Nurse with Bachelor’s Degree, Master’s Degree preferred. Nursing Licensure in the State of Michigan and have a minimum of 5 years of experience as registered nurse. Experience as a care manager preferred.  Background in performance improvement, quality, and population health preferred. JOB DUTIES:  The Behavioral Health Home Team Lead is responsible for the oversight of the Behavioral Health Home (BHH) services provided at Northpointe. BHH services provide integrated, person-centered, and comprehensive care to eligible beneficiaries to address the complexity of comorbid physical and behavioral health conditions. This position will be responsible for assisting with program and team development, care coordination, health promotion, comprehensive transitional care, individual/family support, referrals to community providers, and ongoing monitoring of compliance to meet performance metrics, etc.

The Registered Nurse (RN) coordinates individual’s physical, emotional and mental health care needs.
